AFFINITY DENTAL 401-K RETIREMENT PLAN Contribution, Match and Other Plan Policies
- The Plan provides for employee salary deferral contributions of a percentage of annual compensation subject to maximum tax-deferred limitations established by the Internal Revenue Code.
- Participants who have attained the age of 50 before the end of the plan year are eligible to make catch-up contributions in addition to their regular deferral contributions.
- The Plan provides for employer safe harbor matching contributions for all employees who have attained 12 months of service.
- Employer safe harbor matching contributions are calculated based on 100 percent of the participant’s contribution up to 3 percent of their eligible compensation and 50 percent of their remaining contribution up to 5 percent of their eligible compensation.
- Participants are immediately 100 percent vested in employee salary, rollover contributions, safe harbor matching contributions and any income or loss thereon.
- Vesting in the Company’s employer discretionary profit-sharing contribution portion of their accounts, plus actual earnings thereon, is based on years of service.
- Participants vest in employer profit-sharing contributions made by the Company 20 percent after two years of service and an additional 20 percent per year thereafter until they become fully vested after six years of service.
