GOODMAN CAMPBELL BRAIN AND SPINE 401(K) PROFIT SHARING PLAN Contribution & Match Policy
1. Employee Contribution Rates: Employees can contribute up to 100% of their eligible annual compensation, subject to the maximum annual amount permitted under Section 402(g) of the Internal Revenue Code (IRC).
2. Employer Match/Contribution Conditions: The employer matches 50% of the first 6% of compensation that each participant contributes to the plan.
3. Vesting Schedule/Eligibility: Participants are immediately vested in their contributions and the employer's matching contributions. Vesting in the employer’s profit-sharing contributions is based on years of service: a participant becomes 20% vested after two years of service, increasing by 20% per additional year, with full vesting after six years of credited service. Eligibility for the employer’s matching and profit-sharing contributions requires at least one year of service.
